Karnataka became the second Indian state to enact a policy that enables terminally ill patients to stop life-supporting treatments after following medical assessment procedures. Health Minister Dinesh Gundu Rao has declared this policy progression for the state. The policy provides end-of-life care decision-making abilities to patients which brings relief to patients who suffer from incurable diseases and persistent vegetative states.

The court decision accepts the Supreme Court 2018 directive that death with dignity forms part of Article 21 fundamental rights under the Indian Constitution. The policy provides regulations for discontinuing life support by stating four specific criteria where doctors can perform such actions including brainstem death diagnosis and medical assessments confirming terminal disease conditions without sufficient response to aggressive treatments.

Two medical boards including the Primary Medical Board and the Secondary Medical Board need to approve the withdrawal of life support measures through qualified medical practitioners. Under the Advance Medical Directive (AMD) known as a living will patients can determine their treatment choices in case they become unable to express themselves through incapacity. The introduction of this policy represents a notable initiative that defends patient decision-making rights and preserves dignity throughout end-of-life treatment.

The move has received widespread expert endorsement because Dr Harshal R Salve from AIIMS in New Delhi affirmed that this change provides substantial benefits to families together with patient peace and personnel support. The Director of Critical Care at Fortis Escorts Heart Institute Dr Rajat Agrawal explained that patient autonomy leads to reduced suffering and pain both for patients and their families.
